Police Ombudsman of Northern Ireland (PONI) report details the investigation into the IRA attack.
12 people were killed and 30 injured in the bombing of La Mon House in February 1978 .
PONI has been unable to explain why records from the original police investigation were lost.
The missing documents have given rise to claims police protected some suspects because they were informants.
Former BBC News NI journalist Mervyn Jess was one of the reporters to attend the scene.
He said there was "no panic, no shouting or screaming" at this point.
"All I could see was blackened, charred debris," he said.
Robert Murphy was sentenced to life imprisonment for manslaughter in 1981 and released in 1995 .
