Dr Sara MacRae killed herself in her room at the Royal Edinburgh psychiatric hospital.
Her son Christopher said he gave nurse 'clear evidence' his mum planned to take her own life.
He confiscated evidence and pleaded with nurse to search her room for more dangers.
But the search was not carried out and she died in March 2020 .
NHS Lothian has apologised to the family and upheld complaint about her death.
A fatal accident inquiry (FAI) is currently being considered at the FAI.
Christopher Rzeznicki told FAI it was his error of judgement’ not to take Christopher ’s warnings more seriously.
Previous attempts by Dr MacRae to end her life by a similar method were “not readily accessible in the case notes’ and not known’ by medics charged with looking after her.
A safety briefing meant to be prepared for nurses working on the ward the night Dr Macrae died was not completed that day as expected.
