Bereaved families say no staff members at Nottingham University Hospitals NHS Trust have been disciplined over maternity care failures.
Almost 2,000 cases are being investigated by an on-going review the UK 's largest ever into NHS maternity care which began two years ago and is being led by senior independent midwife Donna Ockenden .
Over 100m has been paid out by NUH for incidents that took place between 2006 and last year .
The review is looking at cases where either a baby or mother died or suffered severe harm.
Ockenden maternity review was established by NHS England in May 2022 .
It replaced an earlier regional review criticised by some families who wanted an independent review.
The midwife had previously led the review into the Shrewsbury and Telford Hospital Trust maternity scandal.
She is expected to publish her findings on the Nottingham trust by the end of 2025 .
