GPs have also been told they can abandon modern methods of referring patients to hospital.
Guidance from British Medical Association admits this could cause delays or mean hospitals refuse to take referrals at all.
BMA guidance advises using an electronic referral system for two-week wait appointments to prioritise urgent cases.
NHS England has preparations in place to ensure patient safety is protected and patients should still come forward for care as usual.
A DHSC spokesman said: “Since the BMA balloted, the Government has accepted the independent pay review body recommendation of a 6 per cent uplift in GP pay.
